Use this questionnaire to find your route to registration with the GDC.

This questionnaire is applicable to dentists and dental care professionals who have never been registered with the GDC.  If you have been registered as a dentist before and wish to restore your name to the Dentists Register, you will need to follow separate procedures for restoring your name to the register.

Dental nurses, dental technicians, clinical dental technicians and orthodontic therapists can also use the questionnaire to find out their eligibility for registration which began on 31 July 2006.
