Dental care professionals (DCP) application forms and guidance

We have recently created new application forms to simplify and clarify the registration process for dental care professionals (DCPs) and their referees. However, if you have one of the original application forms you can still use this.

You can also request an application form by emailing your name and address to gdcregistration@gdc-uk.org  or calling us on 020 7887 3800. Please note that due to the high volume of requests for registration application packs, it may be up to 2 weeks before you receive your application pack.
