Restoration to the Registers

If your name has been removed from the Registers in March 2008 because you have not paid your annual retention fee, you will need to apply to restore your name before you can continue to practise.

To restore your name you will need to complete a form and pay an additional restoration fee.

You can download the forms using the links below. Only use these forms if your name has been removed from the Registers in March 2008 because you have not paid your annual retention fee for 2008. If your name has been removed from the Registers at any other time or for any other reason, click here for information about how to restore your name.

If you are a dentist, please also provide details of the number of CPD hours you have done in 2007 (both verifiable and non-verifiable) with your completed restoration form.
